Every note, tape, photograph and file a survivor picks up goes into the Codex. Signed in, this is your survivor's own journal, read from the game. Records you haven't found stay sealed: what's in them is learned out there, not here.
Finish a category for its title and a one-time reward. Find all seven for The Archivist.
How records turn up
Picked up in the world, earned in the story, heard on the radio with the right parts, taken off something dangerous, or learned the first time you kill something new. A photograph can fill a page in too, if you show it to the right person.

Some pages have a plan tucked into them. Study it from your inventory to learn the recipe.
